Each evaluation begins with a structured clinical interview with Dr. Erin Cook, along with review of developmental, academic, and behavioral history. Teacher and parent input is incorporated when appropriate.
2. Cognitive & Behavioral Assessment
Depending on referral needs, standardized measures may include the WPPSI-IV, WISC-V, or WAIS-5, along with validated behavioral rating scales assessing attention, executive functioning, and related concerns.
3. Integrated Interpretation & Report

We offer two primary types of assessment: an ADHD & Executive Function Evaluation and Cognitive Testing for School or Gifted Placement. The ADHD evaluation is appropriate when there are concerns about attention, executive functioning, learning, or behavioral regulation that may benefit from diagnostic clarification. The cognitive evaluation is intended for private school admissions or gifted and advanced academic placement decisions. What does the ADHD evaluation involve?

The ADHD evaluation is designed to clarify concerns related to attention, executive functioning, learning, and behavioral regulation.
The appointment begins with a clinical interview with the parent and child, lasting approximately 30–60 minutes. This portion is used to gather developmental, academic, and behavioral history and to clarify the referral question. For children under 16, a parent or guardian is required to participate in this interview. For adolescents ages 16 and older, parents should plan to attend the first 15–30 minutes.
Following the interview, the child or adolescent completes standardized testing, which typically takes 1–2 hours depending on age and the measures administered. This may include cognitive testing (such as the WPPSI-IV, WISC-V, or WAIS-5 when appropriate), as well as additional measures of attention and executive functioning when indicated.
Parents do not remain in the testing room during this portion of the evaluation, as standardized administration conditions are required to ensure valid and reliable results.
What does cognitive or gifted testing involve?

The appointment begins with a brief review of completed background forms and relevant records, typically lasting 10–15 minutes. This is used to clarify referral questions and ensure appropriate test selection and is not a full clinical interview.
The child or adolescent then completes standardized cognitive testing, which typically takes 2–3 hours depending on age and the measures administered (WPPSI-IV, WISC-V, or WAIS-5 when appropriate).
Parents do not remain in the testing room during this portion of the evaluation, as standardized administration conditions are required to ensure valid and reliable results.
How long does it take to receive results?
