Exposure and Response Prevention (ERP) Therapy for OCD in Virginia

What Is Exposure and Response Prevention (ERP)?

Exposure and Response Prevention (ERP) is the most effective psychological treatment for Obsessive-Compulsive Disorder (OCD). ERP is a specialized form of cognitive behavioral therapy that helps individuals break the cycle of intrusive thoughts, anxiety, and compulsive behaviors that characterize OCD.

Many people with OCD feel trapped in a loop: an intrusive thought appears, anxiety rises quickly, and a compulsion temporarily reduces that anxiety. Although the relief feels important in the moment, compulsions ultimately reinforce the cycle and keep OCD going.

ERP therapy helps individuals gradually face the thoughts or situations that trigger anxiety while learning to resist the compulsive behaviors that maintain OCD. Over time, the brain learns that the feared outcome does not occur and that the anxiety can naturally decrease without performing a ritual.

This process allows the cycle of OCD to weaken and eventually lose its hold.

What ERP Therapy Looks Like

ERP therapy is structured, collaborative, and tailored to each individual’s symptoms and goals.

In treatment, we work together to:

• identify specific obsessions and compulsions
• understand the patterns that maintain the OCD cycle
• gradually approach feared thoughts or situations in manageable steps
• practice resisting compulsive behaviors or mental rituals
• build tolerance for uncertainty and discomfort

ERP is done at a pace that is challenging but supportive. Many individuals find that as they begin practicing exposures and reducing compulsions, their anxiety decreases and their sense of control increases.

Common OCD Symptoms ERP Can Help Treat

ERP therapy is effective for many forms of OCD, including:

• intrusive or unwanted thoughts
• fear of harming others
• contamination fears and excessive washing
• checking behaviors
• need for things to feel “just right”
• religious or moral obsessions (scrupulosity)
relationship OCD
postpartum intrusive thoughts
• mental rituals such as reviewing, analyzing, or seeking reassurance

Many individuals with OCD are high-functioning and successful in other areas of life, yet still feel deeply trapped by the anxiety and doubt OCD creates.

ERP helps individuals learn new ways of responding to these thoughts so they no longer dominate daily life.

ERP is also highly effective for individuals experiencing postpartum OCD and intrusive thoughts after childbirth.
