Red Elm Psychotherapy welcomes referrals from physicians, therapists, psychiatrists, pelvic floor physical therapists, lactation consultants, doulas, birth professionals, IOP/PHP programs, and other care providers.
We provide specialized therapy for adults experiencing OCD, anxiety, intrusive thoughts, and perinatal mental health concerns. Our work is especially well-suited for patients whose symptoms are persistent, distressing, reassurance-resistant, or outside the scope of general supportive care.
We offer telehealth across Virginia, with limited in-person availability in Vienna.
When to Consider Referring
A referral may be appropriate when a patient is experiencing:
intrusive thoughts that feel frightening, shameful, or difficult to dismiss
repeated reassurance-seeking that does not seem to provide lasting relief
checking, Googling, mental review, confessing, avoidance, or rumination
anxiety that remains intense despite medical reassurance
OCD symptoms during pregnancy, postpartum, or parenting
fear of harm, contamination, illness, morality, relationships, or making the “wrong” decision
panic, hypervigilance, or distress after a difficult pregnancy, birth, NICU experience, or reproductive loss
high functioning on the outside while privately feeling consumed by anxiety or obsessive doubt
need for outpatient follow-up after IOP, PHP, hospital-based care, or a higher level of support
Many of the patients we treat appear highly capable and organized, but internally feel trapped in loops of fear, responsibility, guilt, uncertainty, or “what if” thinking.

OCD and Anxiety
Dr. Niles Cook specializes in OCD and anxiety treatment for adults, including Exposure and Response Prevention therapy. He works with clients across the OCD spectrum, including harm OCD, relationship OCD, scrupulosity, checking, contamination fears, mental compulsions, reassurance-seeking, and obsessive doubt.
ERP helps clients gradually reduce compulsions and build a different relationship with anxiety, uncertainty, and intrusive thoughts.
Perinatal and Postpartum Mental Health
Dr. Erin Cook specializes in perinatal mental health, including anxiety, intrusive thoughts, postpartum OCD, pregnancy-related anxiety, high-risk pregnancy stress, pregnancy after loss, and postpartum adjustment.
She works with patients who may be struggling with scary or unwanted thoughts, intense worry, birth-related distress, rage, guilt, panic, or the feeling that they are “not themselves” during pregnancy or postpartum.

Consultation and Availability
We offer telehealth therapy across Virginia and limited in-person sessions in Vienna. During consultation, we assess fit, clinical needs, location/licensure considerations, and whether our services are appropriate for the patient’s concerns.
If a patient appears to need a higher level of care, crisis support, medical evaluation, or a different specialty, we will communicate that clearly and help direct them toward an appropriate next step when possible.
